The Extended Hours Center in Lake Success, will be closed on Thursday, October 16, Friday, October 24 and Friday, October 31. Regular Extended Hours operations will resume the following day.

As of September 15, 2025, Plainview Cardiology, Gastroenterology, Primary Care at 1181 Old Country Rd and Primary Care at 789 Old Country Rd have relocated to a new, state-of-the-art multi-specialty center at 1600 Old Country Road in Plainview.
