A new Patient Portal has arrived

The CareMount Medical Patient Portal has been replaced. You are able to re-enroll in the brand new Patient Portal. This new portal has an easy-to-use registration process and is only available to Optum Medical Care (formerly CareMount Medical) patients. The ability to communicate with your provider, view upcoming appointments and request prescription refills continues on the new portal.

Important Dates:

November 8, 2022:

The old CareMount Medical portal is no longer active as of November 8th, but you will be able to read your documents in the old portal until mid-December. Starting today, please make any requests or download any relevant documents that you may want to save.

The last two months of your medical history will carry-over to the new portal when you enroll, and you can request more of your health information after logging-in. Additionally, messages or prescriptions will not be able to be sent or received as of November 8. When you re-enroll for the new portal, these features will then become available again.

November 16 – 25, 2022:

Re-enrollment in the new portal is easy. During this time, you will receive an enrollment email with a sign-up link. This link is your access to sign-up and register for the new portal. The portal invitation will come from [email protected] with the subject line “You’re invited to create a Patient Portal account at Optum, formerly CareMount Medical.” If you don’t see the enrollment email by November 25, please check your junk folder just in case!

As soon as you received your invitation, please register for the new portal! Once you have signed up for a new account, you will have 24/7 access to our convenient online services.
